PRISM User Privacy
Policy
This User Privacy Policy applies to your use of Promoting Resiliency in Stress
Management (PRISM), a mobile software application created for Seattle Children’s by General
UI to help teens and young adults develop skills in stress management and
mindfulness, goal setting and problem solving, positive re-appraisals of
stressors, and meaning-making and benefit-finding. This User
Privacy Policy explains what information about you is collected by PRISM, how
that information may be used, with whom it may be shared, and your choices
about such uses and sharing.
Seattle Children’s treatment of individually identifiable protected health
information is governed by our Notice of Privacy Practices.
What Information does
PRISM Collect?
User Provided Information
Personal Information. When you download and use PRISM, you may enter personal
information such as: (a) profile information – your name, location and photo;
and (b) your responses to modules contained in PRISM.
Automatically
Collected Information
Aggregate Use Data. Your and other users’ collective use patterns, including
which modules and resource links you access, the time of day you access it, and
the duration of your use are collected and shared with Seattle Children’s, via
a third party, to help Seattle Children’s better understand your experience
using PRISM and how PRISM can be
improved.
Cookies. PRISM does not utilize cookies, which are small text
files that are stored on your computer or mobile device and used to maintain
login preferences of the browser session.
Mobile Device Information. PRISM
does not collect information about your mobile device – type of device, device
ID, IP address, internet browser used or operating system.
How is Information
Collected by PRISM Used?
Seattle Children’s will have access to
(a) your responses to surveys and feedback forms; and (b) aggregate use data
relating to use patterns that is automatically collected. This information will
be used to determine how users interact with PRISM to help Seattle
Children’s better understand your experience using PRISM and how PRISM can be improved.
With Whom is Information Collected by PRISM Shared?
Seattle Children's wants you to understand when and with whom it may share your
information collected by PRISM. Seattle Children’s will not sell or rent
information collected by PRISM to others. Seattle Children's does not share information
collected by PRISM with others except as indicated below.
Service Providers. Third-party service providers who perform services on
behalf of Seattle Children's may access use data from PRISM to allow better
service to you and other users.
Other Situations. Seattle Children's may share
your information in response to a subpoena or similar investigative demand, a
court order, or a request for cooperation from law enforcement or any
government agency; to establish or exercise its legal rights, including, but
not limited to, efforts to investigate, prevent, or take other action regarding
illegal activity, suspected fraud, or other wrongdoing; to protect and defend
the rights, property, or safety of Seattle Children's, its patients, employees,
or others; to enforce the PRISM terms and conditions or other agreements and
policies; or as otherwise required by law. In such cases, Seattle Children's
may raise or waive any legal objection or right available to it.
Is Information collected by
PRISM used by your Health Care Team?
No.
Neither User Provided Information nor Automatically Collected Information in
PRISM will be provided to your healthcare team and neither is part of your
medical record.
How is Your Personal
Information Collected by PRISM Protected?
Seattle Children’s does not have access to your personal information, which is
stored only on your device. Seattle Children’s
only has access to the use data and responses to surveys and feedback forms described
above. Please
be aware that, although we endeavor provide reasonable security for information
we process and maintain, no security system can prevent all potential security
breaches.
What Choices Do You
Have About Collection and Use of Information by PRISM?
If you install and use PRISM information will be collected as described above.
You can stop all collection of information by PRISM easily by discontinuing use
of PRISM or uninstalling PRISM. You may use the standard uninstall processes as
may be available as part of your mobile device or via the mobile application
marketplace or network.
Is your Information
Collected by Third-Party Websites?
When using PRISM you will be (1) directed to a survey offered via the
SurveyMonkey website to provide comments and feedback to help Seattle
Children’s better understand your experience using PRISM and how PRISM can be improved; and (2) offered links to resources
contained on websites of other outside organizations. Seattle Children's is not
responsible for the privacy practices of these organizations. Please
familiarize yourself with the privacy policies of any website you visit to
determine what information is being collected.
Changes to this PRISM User Privacy Policy
Seattle Children's may change this PRISM User Privacy Policy from time to time.
We
will notify you of any changes to PRISM User Privacy Policy
Consent
By downloading and using PRISM, you are consenting to our
collecting, storing, deleting, using, combining and disclosing your information
as set forth in this PRISM User Privacy Policy now and as amended by us.
